Thank you for scheduling your first appointment with us here at Prevail Physical Therapy.
We are grateful that you have chosen us for your service of care!
Before you arrive for your initial evaluation, please follow these steps below.
- Call your insurance and check your in network or out of network physical therapy benefits. If you’re unfamiliar with insurance, click here.
- Go to your Mail
- Open your confirmation email confirming the date and time of your first appointment.
- Click the link on your email and fill and sign the online Intake Form. If you did not receive one, you can fill it out below.
- Open the second email and fill and sign the online form for the Outcome Measurement.
- If you prefer to fill out the paperwork by hand, please print out the eHIPAA intake paperwork, and injury screen forms listed below and send the finished versions back to us at support@prevailpt.com
- In addition, email us any relevant medical information including referrals, prescriptions, x-rays, physician notes, etc.
- Review the directions to our facility, and let us know if you have any questions.

What is the cost, and how are payments made?
If we are using your health insurance benefits, then your insurance company will dictate reimbursement based on the evaluation and treatment procedures performed each visit. As a rough estimate, use $125 per visit, so if you have a 10% co-insurance, then your cost will be $12.50. Every insurance plan is different, so please understand your particular benefits. Co-insurance costs will be billed to you after insurance reimbursement. Copays are collected at the time of service. Deductibles may increase your up-front cost.
Many of our clients opt to pay out-of-pocket for their services. We can supply the necessary documentation for you to provide your insurance company for possible reimbursement if we are not contracted with your insurance company. Our cash payment rate is $130 per hour. Sponsored groups may receive a 25% discount on cash services. Payment plans are available if agreed upon before starting a course of therapy, otherwise, payment is collected at the time of service. Cash, check and credit cards are accepted as forms of cash payment. A Ten-visit package can be pre-purchased for $1,000.
Health Savings Accounts (HAS) and Flexible Savings Accounts (FSA) may be utilized for physical therapy services, co-pays, co-insurances, deductibles, etc.
What to expect on my first appointment?
Before your appointment we will communicate via email or telephone, to discuss;
- Scheduling
- Payment/insurance information
- Demographic intake information
- Facility access
- The issues that you need help with and your goals
Please arrive ten minutes early to your first appointment to finish up any remaining paperwork not completed beforehand. Your therapy time will be more efficiently utilized during the first visit if all of your initial paperwork is complete before your appointment time. We can also optimize time if you procure any relevant imaging reports or medical notes and send them to us before your first visit, as well write via email, a narrative regarding your current issues, ideally with a history of other injuries and issues, before your first visit.
Bring appropriate dress: shorts for lower extremity issues (and appropriate footwear), and tank top/T-shirt for upper extremity issues. Plan on at least 60 minutes for each visit.
Are specialty services covered by insurance?
Specialty services are typically covered by normal health insurance physical therapy benefits, if they are an indicated part of a logical treatment plan and meet the definitions of medical necessity.
How much does it cost to use the AlterG Antigravity Treadmill?
For clients who are participating in an active course of therapy with us, the AlterG use is free. We have time rental options available for non-clients, click here for more information.
